Indications/Uses
Pain relief of trigeminal neuralgia only in periods of exacerbation of true or primary trigeminal neuralgia. Adjunct w/ other anti-epileptic drugs for partial seizures w/ complex symptomatology, as an alternative medication in patients w/ generalized tonic-clonic seizures; monotherapy or as an adjunct to lithium in the treatment of acute mania or prophylaxis of bipolar (manic-depressive) disorders in patients who are resistant to or intolerant of conventional antimanic drugs.
Dosage/Direction for Use
Epilepsy Adult Initially 100-200 mg once or bid, gradually increased at wkly intervals by increments of 200 mg. Maintenance dose: 0.8-1.2 g daily in 2-4 divided doses or up to ≥1.6 g. Childn Usual dose: 10-20 mg/kg. 10-15 yr 600 mg-1 g; 5-10 yr 400-600 mg; 1-5 yr 200-400 mg; up to 1 yr 10-200 mg. Doses are divided & given by 2-4 x daily. Trigeminal neuralgia 10 mg once or bid, increased gradually. Usual maintenance dose: 400-800 mg daily in 2-4 divided doses or up to 1.6 g daily may be required. Prophylaxis of bipolar disorder 40-600 mg in divided doses.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to carbamazepine, or any tricyclic compds eg, amitriptyline, imipramine, nortriptyline, trimipramine. Should not be administered to patients w/ history of hepatic disease. Should not be used immediately before, in conjunction w/ or immediately after MAOI.
Special Precautions
Patients w/ mixed seizure disorder including atypical absence seizures. W/draw at once if there are signs & symptoms of TEN (Lyell's syndrome) & SJS. Pregnancy & lactation.
Adverse Reactions
Drowsiness, dizziness, unsteadiness on the feet, nausea & vomiting, allergic skin reactions. Lyell’s syndrome, SJS, urticaria, photosensitivity reactions, exfoliative dermatitis; leukopenia, agranulocytosis, aplastic anemia, thrombocytopenia, leukocytosis, acute intermittent porphyria; HTN or hypotension, arrhythmias, bradycardia, CHF, aggravation of CAD; pulmonary hypersensitivity characterized by fever, dyspnea, pneumonitis or pneumonia; diarrhea, constipation, anorexia, glossitis, stomatitis; muscle pain or cramp, arthralgia.
